Dr Swat’s DPC Membership follows the Direct Primary Care (DPC) Model.

DPC is NOT medical insurance. It is a membership model paid monthly that provides patients unlimited access to their physician.

The monthly membership fee will be automatically deducted from the account of your choice. In return, patients have access to Dr. Swat and the services she provides. The DPC Membership does not include the cost of medication. There are NO extra fees regardless of how many services Dr. Swat provided. In addition, Dr. Swat is also practicing Functional Medicine and strives to find the root of cause first before utilizing a pharmaceutical approach. But if a pharmaceutical approach is deemed necessary then Dr. Swat will be able to prescribe any needed medications.

Do you need additional insurance? Additional insurance is recommended to cover labs, imaging, specialist consultation, prescriptions, surgeries or hospitalization.

However, Dr. Swat will also be able to secure WHOLESALE costs for labs, imaging, and medications without insurance if desired.

The purpose of the DPC Membership program is to be able to build personal and one to one relationships with the member. Remember, as mentioned above, the membership program can be canceled at anytime. In addition, since Dr. Swat practices allopathic and functional medicine, so Dr. Swat can treat any acute, chronic or wellness condition without needing to be on the waiting list to see your primary care provider.

Initial One Hour Consultation: Cost $150

The initial consultation includes a complete history and physical, laboratory interpretation and evaluation, vitamin and supplement counseling, and growth hormone optimization.

Weight loss and other services are also available if interested.

If you need to cancel your appointment, we respectfully require at least 24 hour notice. If you do not cancel 24 hours prior or are a no show, you are subject to a $25 cancellation fee.

The membership fee is non-refundable, but you may cancel at any time.
